Case - Shock Differentiation - Case 5 - Cardiogenic Shock - MR - IVR

IVC

The IVC was distended with no respiratory variation.  This was expected given the patient's right heart failure and tricuspid regurgitation.

In this context, if the IVC was collapsed and/or exhibiting significant respiratory variation, it would suggest hypovolemia as the primary mechanism and volume expansion should be considered.

However, given the right heart failure, the IVC could be distended and non-variable YET still require further volume expansion to improve cardiac output.

More information would be needed to inform management.
