Case 1 - Pleural Space

Indication:

(i) Verification:
Whether the pleural effusion was present or not, PoCUS should reveal that.  Other entities could mimic the findings on CXR.

(ii) Therapeutic Thoracentesis Assessment: If a pleural effusion was present, PoCUS was employed to assess the size of the pleural effusion and, also, the landmark a safe site to perform a therapeutic drain.

Image Acquisition:

Given the patient had a hip fracture rendering any hip movements very painful, the decision was to first assessing the right pleural space at Zone 4.

Interpretation:

Minimal pleural fluid was found in the right pleural space.  Instead, a large mass lesion was found at the base of the thorax.  The appearance of the lesion did not look like consolidated or atelecatic lung.

Clinical Synthesis:


Therapeutic thoracentesis abandoned.  CT chest was organized for which a large mass originating from the pleural space was found.
