Subxiphoid

The most obvious abnormality was the biventricular and biatrial dilatation. 

Given the right ventricular hypertrophy and trabeculations, these findings indicate the process was chronic rather than acute (acute on chronic cannot be differentiated, however).

The atrial septum was not deviated which would suggest the PAWP (≈ LA pressure) > RA pressure.

The left ventricle appeared to be hypokinetic per known from prior.

A small pericardial effusion was noted adjacent to the RA.
